Chemical Collection Request
Instructions:
In order to ensure that the online form is completed properly, please follow these instructions:
- Complete all of the "Generator Information" fields on the initial form.
If any required information is left incomplete, you will receive an error message and the form will not send.
- The second page is simply for you to review and verify all information before making a final submittal. You must press the submit
button at the bottom of this page in order for the form to reach EH&S.
- A third page will appear with the words "Chemical Container Label" at the top. It may take a few seconds for this page
to finish loading. When finished, it should include the list of chemical
constituents that you entered on the first page. This page can serve as the container label, and should be printed off and
attached to the container.
- Using your browser's BACK button may cause errors in submitting the form. To avoid this, please use the buttons and links provided on the form pages.
In most cases, chemicals submitted online will be collected
within 7 working days.
